Struck-by is one of OSHA's Fatal Four: vehicles, swinging equipment, falling loads, flying debris. Half of struck-by deaths involve heavy equipment — and the operator usually never saw the victim.
If you can't see the operator's eyes, they can't see you. Make eye contact before crossing equipment paths. Never walk behind a machine that could back up.
Not to pass through “real quick,” not to guide it from below. Loads drop when rigging fails, and rigging fails without warning.
Class 2 or 3 hi-vis around traffic and equipment, day and night. Faded, filthy vests don't count — the whole point is being impossible to miss.
Tools tethered when working at height, materials stacked stable, loads rigged by qualified riggers. A wrench from 30 ft hits like a bullet.
