Contractor prequalification document checklist generator

Before a general contractor, owner or utility lets your crew on site, they usually run your company through a prequalification process — directly, or through a third-party platform such as Avetta or ISNetworld. Those platforms collect the same handful of document categories in different forms: written safety programs, injury and illness logs, your experience modification rate, proof of insurance, and per-employee training records. This checklist lists what is typically requested, who actually provides each item, and which of our tools already cover it — so you know what to gather and what you still need from your insurance broker or workers’ comp carrier.

What you get

A ready-to-use PDF with your company filled in, version and effective date, a management signature block and an annual review line. Blank lines are left where only your site can answer.

  • The document categories prequalification platforms and GCs typically ask for
  • Who provides each one — you, your insurance broker, your carrier, or BuildRight
  • Written safety program checklist, with links to every generator on this site
  • Where your OSHA 300/300A logs, EMR letter and certificates of insurance come from
  • How per-employee training records and certificates map to your team dashboard
  • JSA samples and toolbox talk / safety meeting records — what we provide, what you keep
  • Practical submission notes: renewal timing, matching your legal company name, responding to document review comments
  • A tracking table to check off each item as you gather it

Questions safety managers ask

Is BuildRight Academy affiliated with Avetta or ISNetworld?

No. We are an independent training provider. This checklist describes, in general terms, the kinds of documents those platforms and most GC prequalification processes commonly ask for, and shows which of our own tools can help you produce your piece of it — training certificates, JSA records, and the written safety program templates on this site. We do not submit documents on your behalf and cannot guarantee a score, grade or approval.

Can BuildRight provide our EMR letter or OSHA 300/300A logs?

No — those have to come from your own records. Your experience modification rate (EMR) letter comes from your workers’ compensation insurance carrier or broker, and your OSHA 300/300A logs are your own injury and illness records, which only you can compile and certify.

Do your certificates work as the "training records" prequalification platforms ask for?

They can be part of it. Every course certificate carries a unique number verifiable at buildrightacademy.us/certificate-check, and a team account keeps a roster and compliance matrix you can export per worker. Some platforms or clients specifically require the OSHA 10/30 Outreach (DOL) card for certain trades — we do not issue those; check what your client actually requires before you submit.

What about our drug & alcohol policy or incident investigation records?

Those stay yours — we do not generate a drug and alcohol program or incident investigation template. The checklist lists them so nothing is missed, with a note on who typically owns that document (your HR/safety policy and your incident records, respectively).

A checklist, not legal or compliance advice, and not affiliated with Avetta, ISNetworld or any other prequalification platform. It helps you track down what each document category usually means and who provides it; the exact requirements, scoring and approval are set by the platform and the client that requires them.
