Does OSHA require a toolbox talk sign-in sheet?
There is no single OSHA form for toolbox talks. But several standards want written proof that people were trained — fall protection is one (29 CFR 1926.503(b) calls for a written certification record) — and most GCs, carriers and insurers ask for signed meeting records. A signed sheet in the site file covers you.
What goes on a toolbox talk sheet?
The topic, date and time, the job site, who ran the meeting, the key points you covered, anything the crew raised that needs fixing, and a line for every person who attended to print their name and sign.
